Elder and Adult Day Services is currently seeking qualified candidates for the following position:
Social Worker
We are seeking a Social Worker for the Bellevue Center for 30 – 40 hours/week; occasional evenings.
Responsibilities and Requirements:
- Conduct, review and monitor comprehensive assessment for all participants.
- Coordinate counseling and education for participants and families.
- Perform and provide oversight for required documentation.
- Provide clinical supervision for case management functions as needed.
- Coordinate treatment and interface with community agencies.
- Provide staff education for social, emotional and cognitive needs of participants.
- Supervise and coordinate training of social service students, interns and volunteers.
- Market program to the community in coordination with the Board of Directors.
- Develop and monitor social services budget.
- Coordinate social services at new program sites.
- Maintain clean and safe work environment.
- Assist with structure program of activities
- Provide beneficial and safe care for participants in the program.
- Perform set-up, closing and activity clean-up duties.
- Perform psychosocial assessment in coordination with multi-disciplinary team.
- Work with Nursing staff, caregivers and other staff on developing plans to manage participant’s behaviors.
Education requirements
BA/BS in Social Work with a minimum of one year experience in geriatrics, mental health, or developmental disabilities
Certified Nursing Assistant (CNA)
We are seeking an energetic, team player with a Certified Nurse Assistant certification to work in an adult day health setting with disabled adults at our Bellevue Center. The position is full-time – 40 hours per week, Monday – Friday.
Responsibilities and Requirements:
- Assist with the structured program of activities meeting the creative, physical and psychosocial needs of the participant.
- Encourage participants in the restoration or continuation of self-care.
- Encourage maximum independence in activities that match the participant's abilities and interests
- Assist RN in evaluating health maintenance needs of participants.
Education requirements
Current Certified Nursing Assistant certificate with a minimum of one year experience working in the fields of mental health, geriatrics, and/or developmental disabilities. Food Handlers permit also required.
